Bank Letters For Recognized Campus Groups at UTSG

Bank Letter requests will be temporarily suspended from June 21 - July 9, 2017. 

Please be patient and request a bank letter on Monday, July 10 when this form will re-open.

You may use the following form to request a bank letter for your recognized campus group, but please read the following instructions before you proceed.

BEFORE YOU PROCEED PLEASE READ CAREFULLY. The Clubs and Leadership Development office will be checking to verify that:

  1. The Primary & Secondary Contact provided in this form is the Primary or Secondary Contact listed for your group on Ulife.
  2. The email address provided in this form is a valid, personal, @mail.utoronto email address that is listed on Ulife for the Primary Contact.
  3. Your group is currently recognized through the 2016/2017 academic year.
  4. The Primary Contact provided in this form has responded to the confirmation email that will be sent to them after this form has been submitted. ALL leaders and signing officers MUST first be listed on Ulife.

If any of those points do not check out, your request will be cancelled and must be resubmitted and confirmed by the Primary Contact.

You will be notified when the letter is read for pick-up.
